Long-Term Care

Capital Living and Rehabilitation Centres (CLRC) offers 24-hour long-term care at all eight of our locations. Over the years, many individuals have selected Capital Living and Rehabilitation Centres to be their home. We pride ourselves in offering a caring, comprehensive, family-centered approach to care. At the time of admission, the Resident and Family Service Director are available to provide counseling to residents and family members to assist with the transition to their new home.

All residents are assessed by members of our clinical team who implement a personalized care plan developed to meet the resident’s individual physical, mental and psycho-social needs. We invite patients/residents and family members to participate in making care-planning decisions through communication with the facility staff and by attending their care-plan meetings which are held quarterly. At Capital Living and Rehabilitation Centres, the resident is the most valued member of the team, and we let them know that through caring, compassion and commitment. CLRC facilities have active and vital Resident Councils. Residents are encouraged to participate in the Resident Council so that they may become active participants in our on-going improvement process.

Long-term care residents who are affected by an acute illness may benefit from our on-site comprehensive rehabilitation program. We maintain a full complement of licensed therapists who specialize in physical therapy, occupational therapy and speech therapy. Residents who do not require rehabilitation services from a licensed therapist receive unit programs or Nursing Rehabilitation programs to maintain their highest level of independent functioning.

For enjoyment, we offer a diverse therapeutic recreation and social activities calendar in which many of our residents participate and enjoy as well as our daily community dining and lovely outdoor social areas.
