Short-Term Care

Capital Living and Rehabilitation Centers offers an alternate option to a rehabilitation hospital for your short term care needs. Our experienced nurses and therapist are ready to meet your need for short term physical, occupational and/or speech therapy, wound care, IV Therapy and education programs. Your program will concentrate on those skills that will allow you to return home safely as soon as possible.

Upon admission a personalized plan of care is developed for each patient’s medical, physical, and psycho-social needs while promoting independence. The team offers a cohesive plan that assists the patient/resident and family members reaching their maximum potential and optimizing their quality of life. We invite patients/residents to participate in family teaching and care planning and discharge planning meetings.

The overwhelming majority of patients who are admitted for short- term rehabilitation are successful in meeting their treatment goals and are able to return home or to independent living.
